Huron-Perth will be one of 24 regions in Ontario to have a Health Team. Huron Bruce MPP Lisa Thompson and Perth-Wellington MPP Randy Pettapiece were in Mitchell on Friday (6th) for the announcement.
According to a news release, the purpose Ontario Health Team will organize and deliver health care that better connects patients and providers in their communities to improve patient outcomes.
It aims to give patients easier transitions from one provider to another, including, for example, between hospitals and home care providers, with one patient story, one patient record, and one care plan.
In a statement, Minister of Health and Long Term Care Christine Elliott says the new health team will play an essential role in delivering the government’s commitment to end hallway health care.
She says this will build a connected and sustainable public health care system centered around the needs of patients.
