A pilot project is underway in Lambton County that will see the Moderna vaccine given to patients with high-risk chronic health conditions in four primary care settings.
The pilot which began this week aims to increase immunizations to eligible patients in the region, build capacity for the delivery of vaccines and expand vaccine distribution channels.
North Lambton Community Health Centre in Forest, Central Lambton Family Health Team in Petrolia, and Twin Bridges Nurse Practitioner Clinic, and Rapids Family Health Team in Sarnia are the sites.
Each provider has gotten a share of 2600 doses to start.
Eligible patients will be contacted to book an appointment.
